Notes:

• Proof of income (e.g., tax return, pay stub, SSI/SSDI letter) are required. 
• All rates cover intake, testing, scoring, interpretation, report writing, and feedback session.  
• Additional services (e.g., expedited reports, school meetings, add-on evaluations, court ordered evaluations) may incur separate charges. 
• Payment plans are available on a case-by-case basis.
